Forms

The following forms can be downloaded if you are unable to register your team or, for a tournament, on-line. The American Youth Basketball Association (AYBA) encourages teams to register on-line, using Pay Pal. However, teams can register by mail, using these forms. In those cases, only money orders or certified checks can be accepted.

• NATIONAL TOURNAMENT REGISTRATION FORM (.pdf)
• NATIONAL TOURNAMENT ROSTER FORM (.pdf)
• TEAM REGISTRATION FORM (.pdf) used to register teams for AYBA membership. Membership is a flat $75 per team, for as many as 15 players. That fee includes $25,000 of secondary accident insurance for all players. The insurance is effective Sept. 1 – Aug. 31, each year.
• TOURNAMENT ROSTER FORM (.pdf) used to register for any AYBA state qualifying or championship tournament. Complete the form and mail to the appropriate State Director with a money order or certified check in the amount of the tournament entry fee. Documents proving players’ age and grade must be attached. This form need be completed ONLY ONCE, BEFORE YOUR FIRST AYBA EVENT. Thereafter, you would only submit this form to document legitimate roster changes.
